Revenue Object 3560 – Medical Examination and Registration – Midwives

To record fees for regulating midwives. The commission shall set fees in amounts to cover costs of adminstration for licenses, renewals, examinations and other programs or activities for which a fee is authorized. Fees may not be set in an amount less than the amount of the fee on 9-1-93.

Fees to renew expired licenses are between 1-1/2 to 2 times the normally required renewal fee.

Related Administrative and Civil Penalties

An administrative penalty may be assessed not to exceed $5,000 per day for each violation. Each day a violation continues or occurs is a separate violation for purposes of imposing a penalty. (collected under Revenue Object 3770).

A civil penalty may be assessed not to exceed $250. Each day constitutes a separate violation. (collected under Revenue Object 3717).

Every two years on March 1.
