Description
To record fees set by the executive commissioner to accompany applications for emergency medical services, including:
- Application for certification of emergency medical technician-paramedic or advanced emergency medical technician, no more than $90.
- Application for certification of emergency medical technician or emergency care attendant, no more than $60.
- Recertification of emergency medical technician-paramedic or advanced emergency medical technician, up to $90.
- Recertification of emergency medical technician or emergency care attendant, up to $60.
- Emergency service vehicle application fee up to $500 per vehicle; vehicle fee no more than $180; valid 2 years.
- Certification of program instructors or examiners, no more than $30, certification of course coordinators up to $60; valid for 1 year.
- Emergency Medical Services Volunteer Providers are exempted from fees.
- Replace a lost certificate is no more than $10.
- If someone fails an exam, to retake all or part requires a fee of no more than $30.
- Application for personal certification and training program approval.
- Application for recertification whose application received after the expiration date of the certificate must pay a late fee of $25.
- Each out-of-state certification application, a fee of no more than $120.
- Each out-of-country certification application, a fee of no more than $150.
- Basic course/training program no more than $30; advanced course/training program up to $60.
- Provisional license if provider fails to meet minimum standards but issuance is in public interest, an application fee of no more than$30.
- For reinspection to determine compliance, a fee of no more than $30.
- Certification or re-certification of a licensed paramedic of $120.
