Description


A person is required to hold a life agent license if the person does not hold a general life, accident and health license and acts as:

  • an agent writing coverage on human lives;
  • an industrial life insurance agent;
  • an agent writing fixed or variable annuity or life contracts;
  • an agent writing in excess of $25,000 on a life for a stipulated premium; or
  • an agent writing any other insurance for consumer protection required by the commissioner. 

The department may charge a fee for an occupational provisional permit.



Due Date


Application and fee due before operating as agent; renewal 2 years from date of issue; fee due at time of application. Department may adopt staggered renewal system.
