Revenue Object 3557 – Health Care Facilities Fees – Convalescent and Nursing Home

(1) Initial, renewal and change of ownership license fees NTE $375 per home plus $15 for each unit of capacity or bed space. (242.034)

(2) Probationary license fees NTE $125 per home plus $5 for each unit of capacity or bed space. (242.034)

(3) Background Examination Fee in an amount necessary to defray the department's expenses for background examinations. (242.034)

(4) When institution hires a new administrator or chief manager, it shall notify the department in writing and pay a $20 administrative fee. (242.010)

(5) In lieu of inspection for license renewal, a nursing home accredited by the Joint Commission on Accreditation of Hospitals may submit a copy of its annual accreditation review in addition to application, fee and report required for license renewal. (242.047)

(6) Fee may be charged for certification that institution provides special care and treatment of persons with Alzheimer's and related disorders. (242.040)

(7) Annual license fee charged if amount deposited to Nursing and Convalescent Home Trust Fund (No. 0992) is less than $10,000,000. Fee is $1 for each licensed unit of capacity or bed space in the home or in amount needed to provide $10,000,000 in fund. Fee may not exceed $20 for each licensed unit of capacity or bed space in a home. (242.097)

(8) Fee may be charged for expedited life safety code and physical plant inspection as set by department rule. (242.039)

(9) Provisional license fee as set by the department. (242.314)

(10) License renewal fee expired 90 days or less is 1 1/2 times normal renewal fee. License renewal fee expired over 90 days is 2 times normal renewal fee. (242.315)

(11) A person who was licensed in this state, moved to another state, and is currently licensed and has been in practice in the other state for the two years preceding the date of application may obtain a new license without reexamination. The person must pay to the board a fee that is equal to two times the normally required renewal fee for the license. (242.315)

(12) Fee set by department in a reasonable amount for review of building plans to construct or modify an institution. (242.0385)

(13) Civil and administrative penalties may also be charged. Includes penalties for violations of statutes, rules or orders adopted or license issued under this Act. Department may charge a penalty NTE $1,000 per day for each violation. (242.064, 242.066) Administrative penalties collected under Revenue Object 3770 and civil penalties under Revenue Object 3717.
