Description

Before engaging in the business of providing home and community support services, the agency or person must acquire a license. Fees are set by the department and may not be less than $600 or more than $2,000 for a license to provide home health, hospice or personal assistance services. If the license expired, renewal set by the Department. A fee NTE $50 is required for a change in applicant information. Late fees related to application changes are set by the department.

The Department may assess an administrative penalty of not less than $100 or more than $1,000 for each violation (collected under Revenue Object 3770). Each day of a continuing violation constitutes a separate violation. A person who operates a home health facility without a license is liable for a civil penalty of not less than $1,000, nor more than $2,500 for each day of violation (collected under Revenue Object 3717).

Due Date

Initial fee at time of application; each license renewable one year from date of issuance.
