Revenue Object 3560 – Medical Examination and Registration – Midwives

Fees charged for the regulation of lay midwives are established by rule by the Board in amounts reasonable and necessary to produce sufficient revenue to cover administration and include:

1. For each training course it conducts
2. Application for examination
3. Annual documentation fee (non-refundable)
4. Late filing fee

Late fee: 90 days or less - fee equal to 1-1/4 renewal fee; 91 days - 1 year - fee equal to 1-1/2 times renewal fee; over 1 year - may not renew, must reapply.
 
Fees may be adjusted but may not be set in an amount less than the amount of that fee on 9-1-93. The Board may assess an administrative penalty not to exceed $5,000 for each violation (collected under Revenue Object 3770). A civil penalty may not exceed $250 (collected under Revenue Object 3717). Each day constitutes a separate violation.
