Description


Fees for each applicant for health maintenance organization (HMO) agent license, for each appointment by an HMO and for agents of single health care HMO service plans. Fees are set by the Department in amounts reasonable and necessary to implement this title. Fees include: Examination Fee, non-refundable License Fee, non-refundable Appointment Fee, Duplicate License Fee, Late Fee and Temporary License Fee.



Due Date


Application due before operating as an agent; renewal 2 years from date of issue. Department may adopt staggered renewal system.
