To record license and renewal fees for providing home and community support services, including home health, hospice, habilitation or personal assistance services. Fees may not be less than $600 or more than $2,625. 

To record a fee required for a change in application information. Fee may not exceed $50.

To record late fees related to failure to report application changes within the time prescribed. Fees set by the executive commissioner by rule.

The commission may assess an administrative penalty of not less than $100 or more than $1,000 for each violation (collected under Revenue Object 3770). Each day of a continuing violation constitutes a separate violation.

The commission may also assess a $500 administrative penalty for violating laws relating to advance directives (collected under Revenue Object 3770).

The commission may assess a civil penalty for operating a home health facility without a license of not less than $1,000, nor more than $2,500 for each day of violation (collected under Revenue Object 3717).

Initial fee at time of application; each license renewable three years from date of issuance.
